Request An Incident Report

The Columbia-Richland Fire Department generates an incident report whenever a fire apparatus respond to emergencies such as fires, hazardous material (HAZMAT) incidents, rescues, vehicle accidents/fires or other perilous situations. Incident reports are public records and can be requested by the owner, occupant, or property manager of the affected property.

**NOTE: Proper identification linking you to the affected property is required to obtain a report. **

Once your request is submitted, the Columbia-Richland Fire Department will process the request and compile the necessary documents. The incident report will be emailed or available for pickup within seventy-two (72) hours of the next business day following your request.     If you are picking up your requested report confirm that it is ready. Reports may be picked up at 1800 Laurel Street (enter through the doors located on the 1700 block of Barnwell St.) Monday-Friday (excluding holidays) between the hours of 9AM - 5PM.

    Incident reports are only available using this form to property owners, renters and occupants. In order to prove you are connected with the property involved in the incident (i.e. the home, business or vehicle) please attach supporting documentation to your request before submitting your form. If the incident involved a home or business supporting documentation can include a copy of your lease or a bill with the property address. If the incident involved a vehicle supporting documentation can include the lease of the vehicle, a registration card or an insurance card. Also please attach a copy of any government-issued photo identification that you have for yourself (i.e. drivers license, passport etc.)
